Uniquely You Learning Center: Terms and Conditions

Please read the Uniquely You Learning Center Waiver and Release of Liability Form below. You can find the form for printing in the Resources tab.

1. Acknowledgment of Services

I acknowledge that my child will be participating in services provided by the above-named learning center. I understand that the sessions may include structured activities, games, discussions, and other forms of interactive engagement. I understand that these services are educational in nature and not guaranteed to result in specific academic outcomes or performance improvements.

2. Voluntary Participation

I acknowledge that participation is voluntary and that, while the program is designed to be safe and beneficial, there may be some inherent risks, including emotional discomfort, social stress, or minor physical activity. I assume full responsibility for any risks arising from my child’s participation.

3. Assumption of Risk

I understand and agree that participation in tutoring sessions, including in-person and/or online sessions, and classes may involve certain risks, including, but not limited to, injury, illness, or property damage. I voluntarily assume all risks related to my child’s participation.

4. Release of Liability

In consideration of being permitted to participate in Uniquely You's services, I, on behalf of myself (and my child, if applicable), release and discharge the tutoring center, its owners, employees, contractors, and affiliates from any and all liability, claims, demands, or causes of action, known or unknown, arising out of or related to participation in the tutoring and class services.

5. Medical Authorization

In the event of a medical emergency while my child is attending an in-person session and I cannot be reached, I authorize the Uniquely You's staff to seek emergency medical care as deemed necessary. I understand I am responsible for all medical costs incurred.

6. Code of Conduct

I understand that respectful behavior is expected from all students. Uniquely You reserves the right to suspend or terminate services without refund if a student’s behavior is disruptive or poses a risk to others.

7. Technology Use (if applicable)

I understand that if tutoring occurs online, it may involve the use of third-party platforms (e.g., Zoom, Google Meet), and I consent to their use for educational purposes. Uniquely You is not liable for data breaches or technical issues beyond its control.

8. Late Fee Policy

I understand and agree to the following late fee terms:

Late Pick-Up: If my child is not picked up within 10 minutes of the scheduled end time, a late pick-up fee of $5 per every 5 minutes after the 10-minute grace period will apply. Late pick-up fees must be paid prior to the next class meeting or I understand that my child will not be allowed to attend until payment is made. I also understand that no refunds will be given for classes missed because I did not pay the late pick-up fees on time.
Late Form Submission: If required forms (including this waiver) are not submitted prior to the start of the first class, participation in the program may be delayed or denied until forms are completed. No refunds are given for missed classes due to forms not being submitted prior to the first class.

Failure to comply with late fee policies may result in termination of services, without a refund being issued.

9. Class Cancelation Policy

Uniquely You Learning Center is committed to providing high-quality, consistent programming for all participants. However, certain events or circumstances may require the cancellation or rescheduling of classes or sessions. By signing this liability release form, you acknowledge and agree to the following terms regarding class cancellations:

Cancellations Due to Weather or Emergency

Uniquely You reserves the right to cancel classes in the event of inclement weather, natural disasters, or other emergencies that may pose a risk to the safety of students, staff, or facilities. Notification of a cancellation will be provided via email, text, or phone call, using the contact information provided at registration. In such cases, Uniquely You will make reasonable efforts to offer a makeup session when possible. However, refunds will not be issued for weather- or emergency-related cancellations.

Cancellations Due to Teacher Absence

If a class must be canceled due to an instructor's illness or emergency and a qualified substitute is not available, Uniquely You will notify participants as soon as possible. A makeup class will be offered at the end of the session. Refunds will not be granted for teacher-related cancellations, except in cases where no makeup session is provided.

Non-Liability for Missed Classes

Uniquely You is not responsible for a participant’s inability to attend a class for personal reasons, including but not limited to illness, scheduling conflicts, travel, or transportation issues. No refunds will be provided for missed classes not canceled by Uniquely You.

Acceptance of Terms

By participating in programs at Uniquely You Learning Center, you acknowledge and accept the possibility of cancellations or scheduling adjustments and agree not to hold Uniquely You, its staff, contractors, or affiliates liable for any inconvenience, lost time, or expenses resulting from such changes.
